UW Digital ID - Customer - OTP Device Locked / Disabled
This document summarizes what a customer needs to do in order to unlock / enable a locked OTP device.
You receive an "Operation not allowed in current state of credential." message
1. If you have locked your device by entering 10 incorrect OTP passcodes, you will now need to contact your LRA
2. To verify your identity you must present a government issued photo ID and your UW ID to an LRA.
Acceptable forms of Identification: UW Campus ID, UW Health ID, Drivers License, Passport, US Military ID Card
3. To find out who your LRA is click on the "Click here for assistance" link at the bottom of the page within the UW Digital ID site.
You can also find this information by clicking on the "Find Help" link in the left-hand navigation menu within the UW Digital ID site.
4. After you have verified your identity, the LRA can then unlock / enable your OTP device
